There is something wrong coming from the mesher. This is hard to debug remotely. I suggest that you try reducing the number of times you run the refinement script. Alternatively, try adjusting some of the meshing parameters.
I remember if I used to run too many refinements in the past, that the mesh output from gmsh would not be simulate.
